If time allows before your first appointment, you will receive an informative brochure and medical/dental history forms shortly after you call us to make your initial appointment. By completing the forms prior to coming to the office, our intention is to have you seeing the doctor shortly after you walk in the door. Reading the brochure will familiarize you with our practice and office. Like this website, it will address many of the questions you may have prior to seeing us.
All of the information that is contained in the brochure, and forms equivalent to those that you would receive in the mail, are available on this Web Site. You can complete your forms via this Web Site and E-mail them to us (New Patient Information Forms) (click the link to access the forms) if you prefer doing so, or, you don't receive the forms in the mail.
Your first appointment will consist of a review of your medical and dental histories and a comprehensive clinical examination. The purpose of this visit is to determine your treatment needs. With the information obtained from this visit we will work with your dentist to customize a treatment plan that is most appropriate for you.
The initial appointment provides an opportunity to discuss issues of concern and a time for the patient and doctor to become acquainted. Please feel free to ask all questions that you may have. We welcome and encourage our patients to take an active role in their therapy and we will make every attempt to keep you fully informed throughout the course of your treatment.
Your first visit will consist of diagnostic processes so that the doctor can evaluate your condition. If time allows, a consultation may occur during the first visit. Often, a second, and sometimes a third visit is required to complete the diagnostic and treatment planning phase of care.
